What is your name?

You must have noticed that there are two different forms in Hebrew for the question What is your (sg. m./f.) name? For example, the question addressed to a man is either מַה שִמְךָ?‏  ma shimHa? or אֵיךְ קוֹרְאִים לְךָ?‏  'eH kor’im leHa? The difference between the two is that the former is considered more formal than the latter.
